Well, the gold standard for diagnosing a herniated disc is a MRI.

Many of our spinal decompression patients come to us with MRI in hand looking for nonsurgical solutions to their problem. Other times, we end up ordering the MRI after conservative chiropractic fails to deliver results.

DRX9000 spinal decompression

Spinal Decompression Therapy with DRX9000

We have been treating disc herniations in San Francisco for over 21 years. About 8 years ago we began to incorporate nonsurgical spinal decompression with the DRX9000 into the mix.

The DRX9000 spinal decompression system is a computerized machine that is designed and built to treat lumbar disc herniations, bulges, stenosis, facet syndrome, and degenerative disc disease without drugs or surgery. The machine we have cost’s over $150,000 because it also treats disc herniations in the neck. There are much cheaper and less effective machines out there so beware.

DRX9000 spinal decompression is the Rolls Royce of spinal decompression systems. We like the machine because it is very comfortable for the patient while at the same time effective. Most of the patients we treat fall asleep during the treatment.

DRX sessions usually last about an hour or less. It takes a few minutes to set you up and harness you so we can connect you to the machine. You are on the machine for 30 minutes or so. Then, we do 10 minutes of electrical muscle stimulation to teach the spinal muscles to work with the spine in it’s new position, and 10 minutes of ice to reduce muscle spasms, pain, and inflammation. Sometimes we will incorporate deep tissue laser therapy if there is a lot of initial pain and inflammation in the beginning.

The DRX9000 treatment protocol is for 20 sessions over a 6 week period. Sometimes it takes more than this. We incorporate exercises at the halfway point. There is extensive home-care instructions including dietary and nutrition recommendations.

Success rates are very high.

How much decompression do you need?

We have been providing spinal decompression with the DRX9000 and DRX9000c to the San Francisco Financial District for over 8 years now to thousands of patients. One of the most important things we have learned is that everyone responds differently.

Spinal decompression therapy is a tool and it’s not a Magic Bullet…it requires weeks and sometimes months of initial treatment to achieve favorable outcomes. And favorable outcomes are not always a complete resolution of symptoms. Mostly, patients are just a lot better than they were and are happy with the treatment…many avoiding surgery.

Initially, the manufacturers protocol calls for 20 session of decompression, followed by 10 minutes of electronic muscle stimulation and 10 minutes of ice. Usually this is good enough. But in some cases it’s not and we need to do more. Usually, we realize this in the beginning and build more visits into the treatment program. We can always adjust up or down as the treatment unfolds.

Once we start some people begin to feel better immediately…some feel worse. And everything in between. It really does not matter how a patient responds when we start it’s where they end up when we finish.

Decompression therapy is physical and the body needs to adapt to these new forces and to the spine and discs moving into a more normal position. As the treatment progresses and we begin to incorporate exercises into the mixes there is more potential for soreness. in fact, patients tell us they feel much better from less nerve pain but the muscles soreness is still there…kind of like the soreness you get from working out…a good sore.

Then, at the end, depending on how you progressed and the nature and severity of your problem, and your overall health, we may want to do some more DRX session or a combination of chiropractic, exercise and decompression.

Disc Bulges and Herniations: Most of Us Have Them

Disc Bulges and Disc Herniations are Common. That’s right. If we were to randomly select 100 people off the street and perform a diagnostic MRI to their lumbar spines, 60%+ would reveal disc bulges or disc herniations.

And most would not even have any symptoms or loss of function. But some would have severe low back pain and sciatica and be unable to function normally.

Heck, 80% or more of us will have a bout of severe back pain at least once in our life. And the #1 predisposing factor to chronic back pain is previous back pain. Back pain is epidemic.

Some experts believe discs are becoming injured all the time and the body is repairing these injuries itself. In fact, research suggests that it is best to wait and there is no real advantage to having QUICK low back surgery when it is determined a person has a herniated disc in the low back. It may heal on it’s own and there are nonsurgical treatments now to speed up the process or help initiate the process when the body is unable to.

BUT…just like the immune system is always battling germs and fighting disease (and some still get sick)…in some people the disc herniations do not repair themselves and they DO cause symptoms and loss of function….sometimes severe.  From my studies this occurs about 15% of the time and still amounts to a very large number of people. WHY?

Well…the discs get lodged and displaced in a certain way that puts pressure on the delicate spinal cord and nerve fibers and the body is unable to fix it. Then, the immune system attacks the area as if it were an enemy in a war over and over to prevent further harm (the inflammation process). The muscles go into spasms to protect the injured disc and restrict the motion that the disc needs to take in nutrients and waste products. The chain reaction can be devastating to some. The condition becomes chronic (over 90 days) and continues to degenerate until the nerve pressure can be removed.

Does this require surgery? Maybe…but maybe not!

There is a treatment called nonsurgical spinal decompression that is done on a machine that is designed and built to repair disc bulges and disc herniations in the low back and neck. The disc decompression machine we use in San Francisco is the DRX9000.

Just like a “respirator” helps the lungs to breath until they can do it on their own, a “spinal decompression machine” does the same thing for the spinal discs. Cervical Disk Herniations: Common Questions

Last month, we discussed the topic of neck pain arising from cervical disk herniations. The focus of this month’s Health Update is common questions that arise from patients suffering from cervical disk derangement.

1. “What can I do to help myself for my herniated disk in my neck?” The mnemonic device “PRICE” stands for Protect, Rest, Ice Compress, and Elevate is a good tool to use in the acute stage of many musculoskeletal conditions.

  • Protect your health by NOT placing yourself in an environment that is likely to harm you, such as playing sports or doing heavy yard work. That is, think about what you do BEFORE you do it and if sharp, radiating pain occurs, STOP and assess the importance of what you are doing. Use the concept, “…don’t pick at your cut.” This means if you want the injury to heal, don’t keep irritating it!
  • Rest is similar. Limit your activities to those that can be done without increasing symptoms, especially radiating pain.
  • Ice – The use of ice reduces swelling/inflammation, which reduces pain and promotes healing. Alternate it every 15-20 minutes (on/off/on/off/on) several times a day. You can also use contrast therapy (Ice/heat/ice/heat/ice) at 10/5/10/5/10 minute intervals to “pump” out the swelling.
  • Compress – The use of a collar worn backwards, if it’s more comfortable that way, can literally “take the load off.” the neck and disks. There are even inflatable collars which are pumped up with air to traction the neck. Other forms of traction will be discussed further.
  • Elevate – The concept of raising the ankle to the height of the heart so swelling can drain out of the ankle is the classic example of “elevation.” In the neck, the traction concept may apply once again.

 

2. “I don’t want to have surgery if I can help it. What can you do as a chiropractor to help me?” This is one of our primary goals, and in fact, the goal of ALL health care providers, even surgeons! Chiropractic offers anti-inflammatory measures: ice, herbal anti-inflammatory agents (ginger, turmeric, bioflavonoid, curcumin, bromelain, Rosemary extract, Boswellia Extract, and more), digestive enzymes taken between meals, muscle relaxant nutrients (valerian root, vitamin D, a B complex, chamomile, magnesium, and others) as well as other non-pharmaceutical options. Treatments consist of manual manipulation, mobilization, traction (for home and office), modalities such as deep tissue laser therapy and low-level laser, electrical stimulation, magnetic field, ultrasound, and others. We also offer nonsurgical cervical decompression with the DRX9000c. Most important is having a “coach” guide you through the stages of healing by first addressing the acute inflammatory stage (first 72 hrs), the proliferative or reparative phase (up to 6-8 weeks), followed by the remodeling phase (8 weeks to 1 or 2 years) and finally, the contraction phase (lifetime – includes the natural shortening of scar tissue). If manual traction reduces neck and arm pain, the use of home traction is very effective. Options include sitting over-the-door traction, laying down versions, and mobile traction collars (discussed previously). Exercises to stretch and strengthen the neck are also very important in reducing neck pain as well as preventing recurrences. If in spite of all the best efforts of this non-surgical care approach should ongoing neurological loss and relentless symptoms continue, we will coordinate care with physiatrists for possible injection therapy and pharmaceuticals, with neurology for further testing (such as EMG/NCV – a nerve test), and/or neuro- or orthopedic surgery – THE LAST RESORT!

Cervical Decompression or Neck Surgery?

We have been providing treatment for cervical disc herniations in San Francisco for over 20 years now. About 8 years ago or so we began to incorporate cervical decompression with the DRX9000c into the mix. Since then we believe we have helped prevent many neck surgeries.

How many times have you heard, “I have a pinched nerve in my neck and have to have surgery.” Though there certainly are cases where surgical intervention is required, surgery should ONLY be considered after ALL non-surgical treatment approaches have been tried first (and failed). It is alarming how many cases of cervical radiculopathy (i.e., “pinched nerve”) end up being surgically treated with NO trial of non-surgical care. Hence, the focus of this month’s article will look at research (“MEDICAL EVIDENCE”) that clearly states neck surgery DOES NOT improve the long term outcomes of patients with chronic neck pain.

Chronic neck pain (CNP) is, by definition, neck pain that has been present for a minimum of three months. This category of neck pain is very well represented, as many neck pain sufferers have had neck pain, “…for years” or, at least longer than three months. Depending on the intensity of pain and it’s effect on daily function, many patients with CNP often ask their primary care provider, “…is there anything surgically that can be done?” The desire for a “quick fix” is often the focus of those suffering with neck pain. Unfortunately, according to recent studies, there may not be a “quick fix” or, at least surgery is NOT the answer. The December 2012 issue of The European Spine Journal reports that spine surgery did NOT improve outcomes for patients with CNP. Moreover, they pointed to other studies that showed some VERY STRONG REASONS NOT to have spine surgery unless everything else has failed. One of the reasons was a higher hospital readmission rate after spine surgery. Another reported that most studies on surgical vs. conservative [non-surgical] care showed a high risk of bias, suggesting the research on surgical intervention was biased in the research approach used. They further reported, “The benefit of surgery over conservative care is not clearly demonstrated.” It is important to point out that the research analyzed studies that included patients with and without radiculopathy (radiating arm pain from a pinched nerve), and myelopathy (those with pinching of the spinal cord creating pain, numbness, weakness in the legs, and/or bowel / bladder dysfunction).

In February of 2008, the Neck Pain Task Force published overwhelming evidence that research supports the use of cervical spinal manipulation in the treatment of both acute and chronic neck pain with or without radiculopathy. Bronfort published similar findings in 2010 in a large UK based study that looked at the published evidence supporting different types of treatment for various conditions. They found cervical spine manipulation was effective for neck pain of ANY duration (acute or chronic). Chiropractic utilizes manipulation, manual traction, mobilization, muscle release techniques, home cervical traction, exercise, as well as a multitude of physiotherapy modalities when managing patients with CNP. Given the overwhelming research evidence that surgical intervention for CNP is NOT any better than non-surgical care, the greater amount of negative side-effects, and the obviously long recovery time post-surgically, chiropractic treatment of anyone suffering from CNP should be tried FIRST.

Spinal Decompression Therapy for Multi-Level Disc Herniations

We have been providing spinal decompression therapy in San Francisco for 9 years now.  The most common symptoms we treat are low back pain, neck pain, sciatica, numbness and tingling, due to herniated or bulging discs in the low back and neck. During this time we have performed almost 20,000 spinal decompression treatments on the DRX9000, the disc decompression system we introduced to San Francisco.

It is not uncommon for prospective spinal decompression patients to present with disc herniations or disc bulges at multiple spinal levels. What happens is, the spine tries to adapt to the first herniation and it just becomes too much for the other spinal discs to handle, and over time they break down and herniate.All the discs are connected and are part of your spinal structure. This becomes a problem when you present to an orthpedic surgeon because they often times have trouble pin pointing which disc is the primary source of the pain and dysfunction. Heck…if you have say three spinal levels of disc herniations, whet are they going to do, operate on all of them? Maybe…but I would not do this if it were me.

This is one of the reasons nonsurgical spinal decompression makes so much sense for multi-level disc herniations. First of all the treatment is drug-less and does not involve surgery. Nothing is cut out. If you remove part of a spinal disc you remove part of your spinal structure. This can make the problem even worse…because the other parts of the spine have to compensate for the loss of structure by absorbing more stresses, predisposing them to disc degeneration, bulges and herniations.

Decompression machines such as the DRX9000 are able to isolate on specific disc herniations one at a time and seek to repair them naturally.

Disc decompression therapy elongates the spine, enlarges the disc space, re-positions the injured disc, rehydrates the disc, strengthens the spinal ligaments, and helps replace the surrounding scar tissue with healthier, more pliable, more normally functioning muscle tissue.

More often than not, patients that present with symptoms such as severe low back pain, sciatica, numbness, tingling, night pain, and many of the other symptoms associated with lumbar and cervical disc bulges and herniations, achieve favorable outcomes and ovoid risky surgery.

Does spinal decompression work every time? NO…just most of the time.

But here’s the deal…If spinal disc decompression fails to deliver results you are still whole and can explore other options. If you have surgery…especially multi-level disc surgery and fusion, you will never be able to try spinal decompression…you will be permanently disqualified. OUCH!

My Advice: Try spinal decompression first. The worse thing that can happen (in the unlikely event that spinal decompression fails to deliver) is you will be out some time and money. But you will still be whole and can explore other treatment options.

Cervical Decompression for C7 Disc Bulge

We have been providing spinal decompression therapy in San Francisco for over 8 years now. The machine we use is the DRX9000c.

Cervical decompression systems are designed and built to treat bulging and herniated cervical discs without drugs or surgery.

The C7 disc bulge is one of the most common conditions we treat with the DRX9000c. The C7 disc is the disc at the very bottom of the cervical spine in between the C7 and T1 vertebra. The C7 disc will often bulge as an adaption to things like abnormal stresses from poor posture over time, injury (sports, auto accident, repetitive strain) or degenerative disc disease…to name a few.

Typically, a bulging disc in the neck will eventually herniate if left untreated. This is why it’s so important to get the right kind of treatment as soon as the disc bulge is diagnosed.

Cervical disc decompression treats bulging and herniated disc by elongating the spine, re-positioning the disc, enlarging the disc space, and creating a negative pressure, which actually pulls the bulging disc material back to the center of the disc.

The treatment protocol is 20 sessions done as follows:

5 times for 2 weeks, 3 times for 2 weeks, 2 times for 2 weeks.

The sessions are about 50 minutes. You receive cervical decompression for 30 minutes, then 10 minutes of muscle stimulation, then 10 minutes of ice.

Bulging cervical discs at C7 can cause symptoms like arm pain and weakness, loss of grip strength, numbness and tingling in the arm and hand, night pain, and headaches.

Success rates are high with the DRX9000c.

How do you know if your spinal decompression is working?

We have been providing spinal decompression therapy in San Francisco for 8 years now. During this time we have performed over 20,000 decompression treatments to thousands of patients. We were the first to offer the DRX9000 in San Francisco.

The protocol for the DRX9000 is 20 sessions. The recommended treatment is 5x for 2 weeks, 3x for 2 weeks, and 2x for 2 weeks. When they conduct research this is what they do.

In reality, not all herniated or bulging disc patients can adhere to this treatment protocol due to family, work or school schedules. And honestly, sometimes we think the treatment might be too much for someone to handle. So…we treat each patient on an individual basis, sometime starting at only 2-3 times per week.

How do we know the 20 spinal decompression sessions will be enough?

We don’t…sometimes it takes more…but it rarely takes less. Sure, if you are a young adult (under 25 years old) and have a single level disc herniation, say at L5-S1, and an otherwise healthy spine, we may recommend 15 sessions.  Other than this scenario, we usually do 20-26 sessions.

In our experience it’s best to complete the program…even if you are feeling better after say 10 sessions. It’s kind of like when you take antibiotics…you feel better before the problem is fixed.

We often have patients that want to only do 10 sessions to see how they feel…but honestly…even if you are not feeling results, it does not mean the decompression therapy is not working. Some of our most dramatic results have happened at the end of treatment…in the last 5 sessions…and the results have lasted years.

And you should never stop after 10 sessions if you are feeling better…because you may risk a relapse.

What we recommend is deciding before you start that you are going to complete the program. Sure, we can split your payments up to make them more affordable…but don’t jeopardize your potential outcome by doing too little…that just does not make sense.

We will do it if you want…but we recommend you “stay the course”.  Because if you do the results will be more permanent. And we have the experience to know. We have seen patients that have done partial treatment get partial results. We we want this to be life changing and long lasting…and so should you!

When to Consider Nonsurgical Spinal Decompression

We have been providing natural solutions for low back pain and sciatica in San Francisco for over 20 years now.  About 8 years ago we began to incorporate nonsurgical spinal decompression with the DRX9000 into our treatment protocols. The results have been outstanding.

Prior to spinal decompression we would have to refer out patients that did not respond to our treatment, which consisted of chiropractic adjustments, myofascial release, deep tissue laser, acupuncture, custom exercises, yoga, and traction. We also consulted our chronic back pain patients on plant based nutrition and meditation.

It sounds like a lot…but sometimes it just was not enough and we would need to refer out for surgical consultation and possible back surgery.

But that was pre-spinal decompression. We still have all the tools at our disposal that we used to have, but we also have the DRX9000 spinal decompression system.

Spinal decompression machines are designed and built to treat neurovascular compression syndromes such as herniated and bulging discs, degenerative disc disease, spinal stenosis, and facet syndrome, without drugs or surgery.

The machine is able to isolate of a specific spinal disc, say L5-S1 (the most commonly treated) and initiate healing.
